The word An before certain words has disappeared,making the following word or sentence sound incorrect.?

tabloids and newspapers do not use the word "an" before certain words or sentences,eg,"a historic occasion", "this was a accident" "a interview with", i read these grammatical mistakes everyday and also hear them spoken on tv,it it makes me so angry,especially when it is printed in our broadsheets.Do they not have anyone that can speak or type correct english ?
